Bonus payments and stock options are separate and apart from your base salary. Your cv should mirror the information on the completed application.. Recruiters know the market place and their antennas go up when they see something that might be construed a different way... Less concerning are the "references"...after all...are you going to provide someone who will say something bad about you?
